The Clinique Dentaire du Boulevard must collect the personal information necessary for the provision of dental care using the attached confidential medical dental questionnaire which is presented to you for signature. This one is kept in your dental file, and your dental file is kept on digital media to which access is controlled using access codes. Only healthcare staff will have access to your health information, administrative staff will only have access to the information necessary to carry out their duties (billing, making appointments, transmission to insurance companies).
